How to Refer Kids Needing Services
- Use this card as a screening tool to help you identify systemic problems facing
the patient or patient's family that are having a negative impact on the health or
welfare of the child.
- Once you have identified the issue, make a referral by either calling the agency
listed, providing that information to your patient or patient's family, or
refering the patient to the hospital social worker or case manager.
- If your office does not have a social worker or case manager, see if the
patient has a case manager in the community.
